Primary Veterinarian Information

Please fill this section in if we are not your primary care veterinary hospital. By listing your primary care veterinarian, you are authorizing Godspeed Animal Care to release patient information to the primary care hospital or veterinarian.
PLEASE NOTE: Clients will be charged an exam fee for missed or late cancellation appointments. 
By submitting this form, I hereby authorize Godspeed Animal Care to render medical care for my pet(s) as deemed necessary by the veterinarian. I understand that no guarantee can be given to the outcome of treatments and take it as my responsibility to comprehend any risks involved. I agree to pay for the cost of all services to which I consent to by written or verbal estimate. I understand that a deposit is required before diagnostics and treatments can be initiated and that payment in full is required prior to discharge of patient from Godspeed Animal Care.

Anything less than the utmost respect for our staff and clients alike, will NOT be tolerated. Our staff is the heartbeat of our clinic and their unwavering commitment to excellence ensures that your pets receive the exceptional care they rightfully deserve.

We recognize that bringing your pet to our practice can be an emotionally charged experience, often accompanied by stress. Our dedicated staff is here to support you during these moments, providing a compassionate shoulder to lean on when times get tough.

By signing below, you acknowledge your commitment to respecting our staff and treating them with the same care and consideration you would expect for yourself and your pets. You agree to not engage in yelling, using inappropriate language, or causing harm to our staff in any manner which also includes rude behavior. Should such behavior occur towards a staff member, you will receive one warning, and it will be documented in your medical records. Should a repeat incident happen, your pet's medical records will be emailed, your account deactivated, and the veterinarian-client-patient (V.C.P) relationship terminated.
